All entities that maintain records with protected health information must prominently post on the entity’s website and at the facility detailed instructions for a consumer to:

- Contact the disciplinary or licensing authority for the entity; and

- File a consumer complaint with the Texas Attorney General (https://www.texasattorneygeneral.gov/consumer-protection/file-consumer-complaint) or the licensing authority for the entity.
